Do I need an appointment or can I just walk in?

We do take walk-ins on an urgent care basis, although we recommend making an appointment. Our Providers try to accommodate patients needing a same day appointment. As a walk-in, you may be asked to see any of our available Providers. Making an appointment helps you be seen in a more timely manner. At Foothill Family Clinic, the patient comes first.


What is a Physician Assistant?

A Physician Assistant is a healthcare professional trained and licensed to practice medicine with limited supervision of a physician. A Physician Assistant is concerned with preventing, maintaining, and treating human illness and injury by providing a wide range of services that are traditionally provided by a physician. Physician Assistants conduct physical exams, pap smears, diagnose and treat illnesses, order and interpret tests, counsel patients on preventive health care, perform minor surgeries, and write prescriptions.


What services does the clinic offer?

Lab work, physical exams, stress treadmills, bone density scans, x-rays, minor surgery procedures, EKGs, spirometry, laceration repairs, ultrasounds, IMT scans, IUD insertion/removals, wart removal, vasectomies, circumcisions, cryo surgery, and colposcopies.


How do I contact my doctor or medical assistant?

The best way to reach our staff is to call (801) 365-1032 for the South clinic and (801) 486-3021 for the North clinic. Each provider has a secretary who can help you with prescription refills, scheduling and cancelling of appointments and tests, and setting up referrals. If you need to get test results or have medical questions, please contact your provider’s Medical Assistant. Please leave a message if unable to reach the person you are calling, as they are frequently away from their desks helping other patients. They will return your call as soon as possible.


What should I bring to my appointment?

Please make sure you bring your insurance card, photo ID, and co-pay if it’s required by your insurance. If you are a new patient, please bring a detailed list of current medications or prescription bottles.


What do I do if I need a refill on my medications?

The fastest way to have your prescription refilled is to contact your pharmacy. They will send your doctor the request.


How do I know that my child is up to date on his/her immunizations?

If you are unsure whether or not you child is current with his or her scheduled immunizations, feel free to contact one of our medical assistants. If your child has had his/her immunizations at another clinic, you may schedule an appointment with one of our physicians. Just remember to bring with you any documentation of immunizations you may have.


How do I get copies of my medical records?

To get a copy of your medical records please contact our Medical Records Department or stop by the front desk at one of our clinics' to fill out a records request. Please allow 2 weeks for all requests.


How do I make pay my bill?

You can make a payment in person, over the phone with one of  our billing staff, or by mail.


Can I see a doctor without having any insurance?

If you are a self-pay patient we will gladly see you. We ask that you bring photo ID and a method of payment with you to your appointment. At check-in we will collect a $100 deposit payable by cash, check or credit card. This deposit will be applied toward the final charges for your visit. A discount may apply if paid in full. The actual charges cannot be determined prior to your visit. We cannot quote prices.


Do you accept my insurance?

Foothill Family Clinic participates with most major insurance companies. For the most current and complete information, please contact your insurance company.
